Every question here is fair, and any clinic worth going to should be comfortable answering all of them before you commit to anything.
Write these down and take them with you. It is easy to forget half of them once you are in an appointment, and the answers are far more informative than anything on a clinic's website.
Ask by role, not just by practice name, and ask whether it will be the same person each time or whoever is available. Ask who conducts the initial evaluation and whether that is the same person who handles follow-up.
Ask what license the person seeing you holds and what supervision arrangement applies. Vague phrasing like medically supervised is not an answer. A specific credential is.
Ask for the schedule, not a range. Ask how long a follow-up visit runs and whether visits are in person, remote, or a mix, and who decides which.
Ask what is measured or drawn, when it is repeated, and what would prompt the clinician to change or stop the plan. This is the question that separates monitoring from paperwork.
Ask for the actual process. Who answers, how quickly, and what happens after hours or on a weekend.
Ask what the program covers, what lab work costs and how it is billed, whether nutrition sessions are included, and what a missed visit means. You are entitled to the full picture before enrolling, in writing.
Ask about stepping down, about records going to your primary care physician, and about what continues to be available to you afterward.
Ask directly whether an evaluation here ever ends without enrollment. A clinic that says yes, routinely, is telling you something worth knowing.
